Course Description:

The course examines helicopter rotor systems, controls, performance variables, flight hazards and material characteristics involved in helicopter operations and accidents.  Although Aircraft Accident Investigation is not a prerequisite, it is assumed that the attendee has either completed AAI or has some experience in aircraft accident investigation.

Objectives:  To provide concepts, practical techniques and methodologies essential to rotary wing aircraft accident investigation.

Who Should Attend:  Persons associated in rotary wing accident investigation including manufacturers, operators, associations, insurers, law enforcement, military and government agencies.

Course Description:

Human error is associated with 60 to 80% of all accidents, injuries, and quality defects across a variety of industries including aviation, healthcare, mining and manufacturing. Managing human error is therefore fundamental to maintaining the viability and profitability of any organization.

This intensive 2-day seminar provides training in the application of innovative methods for managing human error that are scientifically derived, empirically tested, and proven in the field. Participants will learn how to turn errors into information, information into knowledge, and knowledge into effective error management solutions.


Attendees will learn how to:

  • Develop effective error management and system safety programs
  • Conduct an applied human error analysis during an accident or incident investigation
  • Utilize the Human Factors Analysis and Classification System (HFACS®) to identify systemic causes of human error;
  • Implement the Human Factors Intervention matriX (HFIX®) to develop focused, data-driven safety programs
  • Develop a human error database and tracking system for monitoring and evaluating intervention programs

  • Textbook on HFACS
  • HFACS/HFIX Workbook including case study exercises
  • Generic HFIX software "Single-user version"

ISO 9001 2008 Internal Quality Systems Auditor (TPECS)

BSIs ISO 9001 2008 Internal Quality Systems Auditor competency–based 3 day course teaches a general understanding of the concepts of the ISO 9001 2008 standard and the principles and practices of effective internal audits in accordance with ISO 19011 2002 Guidelines for Quality and/or Environmental Management Systems Auditing. Experienced instructors explain the clauses of ISO 9001 2008 in detail and guide students through internal audits that are required for a quality management system based on ISO 9001 2008. Students gain necessary auditing skills through a balance of formal classroom tutorials practical role–playing group workshops and open forum discussions. This course is certified by RABQSA to meet the knowledge requirements of Quality Management System certification for Management Systems Auditing. Attendees successfully completing this course receive a Certificate of Attainment for these RABQSA knowledge competency units QM – Quality Management Systems and AU – Management Systems Auditing
